Crohn¡¯s disease (CD) is a systemic granulomatous disease that may involve any part of the alimentary tract. CD is more common in the terminal ileum and right colon, but rare (only 0.5¡4.0%) in duodenum. Nonsteroidal anti-inflammatory drugs (NSAIDs) reduce prostaglandin production by inhibiting the activation of cyclooxygenase enzyme and have usually been used for arthritis and arthralgia. Inhibition of prostaglandin synthesis not only contributes to the generation of ulcers in the upper gastrointestinal tract, but it can also exacerbate mucosal injury in the small intestine and colon and cause reactivation of quiescent CD. Herein, we report on a case of activated CD with acute massive duodenal ulcer bleeding after NSAID use, and we include a review of the relevant literatures.
